Please find our two referral forms below. All our gastroenterologists are taking new patients for consults. We welcome open access endoscopy referrals.

Our Gastroenterologists work collaboratively with GP’s and other health professionals, and are available to discuss any concerns or queries you may have.

We look after all GIUM bookings - services for intestinal ultrasound and FibroScans. These scans are bulk billed on weekdays, we require the referral prior to booking. These referrals are not continuous, and we must be sent a new one for repeat scans.

We use Argus and healthlink, but for quick processing of referrals we prefer these to be emailed or faxed
